Introduction Cardiovascular disease (CVD) is no longer limited to developed countries, it is widespread in developing countries (1,2). Additionally, the rate of increase in CVD in developing countries is twice as high as in developed countries (3). CVD also typically occurs at a younger age in developing countries: for example, about 52 per cent of deaths from such disorders in India occur before 70 yr of age, compared with 23 per cent in established-market economies (4). In the context of this large and growing disease burden, strategies to improve population health in India require consistent efforts to identify and address the real causes of this rapid rise in CVD.
